The Home-to-Home Foundation is a non-profit 501 (c)(3) tax-exempt organization. All of our programs are made possible through the generous support of private donors and by federal and state grant programs. The foundation assists individuals in their transition from home to home by providing increased human contact through volunteer programs. The transition to a long-term facility can be difficult. Changing from an independent life style and the comforts of home can often lead to depression, physical and mental decline. Just an hour or two of human contact and social activity can make a difference in their lives. 

We need your help to accomplish this!  Please consider a donation which will enable us to continue and to expand our programs so together, we can touch the lives of many nursing home residents. Grant News
  • The Watertown Foundation recently awarded the Home-to-Home Foundation a grant to purchase computer equipment.

  •  The CHEST Foundation has awarded a 2008 grant to the Home-to-Home Foundation in  honor of Dr. Richard Silverman's work.

  • The Naugatuck Savings Bank Foundation has awarded a 2007/08 grant for our "Reading for Residents" program.
